Fractional laser technology is actually an improvement of invasive laser technology, which is a minimally i

nvasive treatment between invasive and non-invasive. It's essentially the same as an invasive laser, but with less energy and less damage. The principle is that fractional laser generates tiny beams of light, which can be applied to the skin to form multiple tiny thermal damage areas. The skin will start its self-repair mechanism due to the damage and stimulate the regeneration of keratinocyte collagen, thus achieving the effect of removing spots and beautifying the skin and rejuvenating the face.

Fractional laser, because of its special pattern generator (CPG) laser, can change the light emission mode, through the high focus lens emit 50 microns to 80 microns of the focal spot, and the focal spot scanning up to 6 kinds of rectangular graphics (round, square, rectangle, diamond, triangular, linear) small beams, will apply to the skin and is organized in the water absorption, the formation of several different small thermal damage zone of columnar structure, which in turn caused a series of biochemical reactions, skin respectively suitable for the treatment of different parts and different type of skin, to achieve the different therapeutic effect.

Classification of fractional lasers:

In terms of treatment methods, a fractional laser can be divided into three types: stripping, non-stripping, and micro-stripping. 

Stripping fractional laser: vaporization, ablation type, with gasification zone, mainly CO2 laser (wavelength 10600nm) and Er: YAG laser (wavelength 2940nm).

Non-stripping fractional  laser: non-vaporized, non-ablation type, with micro-thermal damage zone and non-gasification zone, mainly with the wavelength of 1320nm, 1440nm and the latest 694nm lattice laser.

Microexfoliated fractional  laser: the cuticle is intact, and the epidermal layer below it has a gasification zone, mainly including erbium glass laser (wavelength: 1540nm or 1550nm).

All three are traumatic lasers; Generally, a course of treatment requires 3-5 times of treatment, and the effect can be maintained for 6-12 months. After a course of treatment, a single additional treatment may be performed every 3-6 months. In clinical practice, the specific skin conditions of each patient are different, and the cycle and course of treatment are also different.

Currently, there are three kinds of fractional lasers commonly used in clinical practice: CO2 super pulse dot matrix laser, 2940nm dot matrix laser, and 1550/1540nm dot matrix laser. All these three kinds of fractional lasers work on the principle of water absorption in clinical practice, but different wavelengths have different characteristics of water absorption in tissues. Each of the three wavelengths has its clinical indications, and there is no substitution effect. Doctors must have an accurate judgment of the disease before they can choose a reasonable treatment.

1. The absorption of water by CO2 super-pulsed dot matrix laser is a long nail type, which can act on the dermis, penetrate deeply, and carbonization and gasification tissues are faster; For severe skin aging, the overall price is 4000 yuan/time, the pain is relatively obvious during treatment, generally need to be coated with anesthetic drugs; However, depending on the tolerance of each person, there are also some people do not need to apply anesthetics; Under normal circumstances, the treatment of CO2 dot matrix laser takes about 10min and does not require repeated irradiation. However, skin conditions of each person vary to some extent, and doctors will make adjustments according to experience and skin changes during treatment. After treatment, there will be exudation of skin tissue fluid, which needs to be bandaged. The scab will be removed in about 5-7 days, and the treatment interval needs 2 months. The absorption of water by a 2940nm array laser is in the shape of an umbrella, with a very high absorption rate and insufficient penetration. When applied to the superficial dermis, it is absorbed by a large amount of water on the skin surface. Generally, the energy of 2940nm array laser is extremely high, above 3000MJ, or it is difficult to penetrate the epidermis. During the treatment, there were mild to moderate pain and discomfort, which were within the tolerance range. After several minutes of treatment, mild flushing and swelling may occur, and the discomfort after ice compress will be alleviated. Generally, the interval of treatment is about 1 month. Can treat wrinkles, rough pores, dark skin, superficial sunken acne scars, whitening skin, etc.

3. The absorption of water by 1550nm fractional laser is an inverted cone type, which can act on the dermis and superficial layers and has a high peak of water absorption. However, its penetration depth is deeper than the wavelength of 2940nm, and it can reach the dermis rapidly. The gasification effect is far less than that of CO2 super pulse lattice excitation. Generally used for mild skin aging, the price is 2500 yuan/time, the treatment has a slight needle-like pain, generally tolerable, for patients with poor tolerance can be coated with anesthetic drugs; Relatively speaking, the treatment time of erbium laser is relatively long, generally around 20min, because its treatment itself is relatively shallow, and to achieve good effects, multiple irradiations need to be performed on the key treatment sites, which is relatively inefficient. After treatment, there is no need to bandage, about 5 days to remove the scab, interval 1 month to do the second treatment. Can treat the scar, wrinkle, pore bulky, firm skin, stretch marks, chloasma, and so on.

Contraindication of fractional laser and postoperative care:

Generally speaking, as long as the skin is normal, no wound, scab, can do dot matrix laser. But in pregnancy or lactation of this special population, it is not recommended; Theoretically speaking, dot matrix laser can only affect the eyes and exposed skin, but not the fetus. But pregnant women in the treatment process will have tension, pain and another discomfort, laser beauty itself is the icing on the cake, a special period is not recommended to do.
